Job summary

GreenField Terrace in Lancaster, NY 14086 seeks a Licensed Practical Nurse for full-time, 2:45pm–11:00pm shift with every other weekend rotation. A $7,500 sign-on bonus is offered for FT LPNs.

You will administer nursing care under a RN, support CNAs, and actively participate in residents’ direct care to maintain quality of life. Responsibilities include updating ISP, administering treatments, documenting care, and coordinating hospital readmissions.

Qualifications

• Two years' experience in SNF, acute care, or long-term setting
• LPN License
• First Aid Certification for Court/Terrace Required
• CPR/BLS for GHRC for Nurses (RN’s & LPN’s)

Responsibilities

• Assess resident care needs and assist in the development of Individualized Service Plan (ISP) encouraging resident independence and self-esteem.
• Update and communicate ISP plan of care that identifies health problems/needs and goals for each health problem/need.
• Administer and record treatments/medications.
• Document resident care, condition, and response to care.
• Clarify and implement physician orders.
• Determine need to transfer resident to hospital and coordinate readmissions.
• Communicate resident’s condition to physician and responsible party promptly and take action.
• Count medications and record treatments/medications administered.
• Provide direct resident care and guide aides.
• Other responsibilities may be assigned.

Position Summary:

The Licensed Practical Nurse administers nursing care to designated residents under the direction of a registered nurse. Coordinates and supports the activities of the certified nurse assistants and actively participates in the direct care of the resident, by following the specific approaches as stated in the plan of care.

• Other responsibilities may be assigned.
