Job summary

Homestead of Tonganoxie is seeking an LPN to join our caregiving team. You will provide nursing care under supervision and work collaboratively with the nursing leadership to support residents.

The successful candidate will maintain patient records, follow care plans, and contribute to a supportive, patient-centered environment in a community setting.

Qualifications

• Current LPN license in good standing.
• Ability to provide care per care plans.
• Strong communication with patients and families.
• Experience with charting and maintaining clinical records.

Responsibilities

• Provide care to patients per their care plans and coordinate with physicians and staff.
• Confer with physician, patient, family members and staff involved in care.
• Educate patients and families about progress and care.
• Review and maintain clinical records for patient care and progress.
• Monitor compliance with state/federal requirements (infection control, bloodborne pathogens, hazardous materials, fire safety).

As an LPN, you will work under the general supervision of the communities Director of Nursing to:

• Provide a provision of care to patients in accordance with their care plans. May require conferences with physician, patient, family members and other staff involved with patient’s care.

• Communicate with patient and family regarding education and progress of patient.

• Review and maintain all clinical records, which are required for patient’s care and progress.

• Review weekly reports to monitor personal and staff compliance to all State and Federal requirements including blood borne pathogens, hazardous materials, infection control, and fire safety
