Job summary

Amedisys is seeking a licensed practical nurse to join our home health and hospice team in Savannah, GA. This full-time role offers a base pay converted to per-visit compensation, mileage reimbursement, and a range of benefits designed to support patients at home.

Responsibilities include validating plans of care with RNs, collaborating with physicians, educating patients, monitoring vitals, reconciling medications, and documenting accurately during visits.

Qualifications

• One year of clinical LPN experience is required.
• Current CPR certification is required.
• Valid driver's license and reliable transportation are required.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform procedures ordered by physician (e.g. foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
