Amedisys is seeking a Licensed Practical Nurse to join our home health team in Jacksonville, FL. The role focuses on patient-centered care in the home with RN collaboration, medication reconciliation, and essential clinical procedures.

Qualifications

• 1 year of clinical LPN experience.
• Current LPN license valid in the state(s) of assignment.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to meet patient needs and report changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform physician-ordered procedures (e.g., Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation in a timely and accurate manner.

• Complete documentation timely and accurately.

• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
