Job summary

The Avenue at North Ridgeville, a post-hospital rehabilitation and skilled nursing facility, is seeking a dedicated LPN to provide direct nursing care to residents and supervise nursing assistants. You will ensure care aligns with federal, state, and local standards while delivering compassionate support in a comfortable setting.

Typical duties include monitoring residents, administering medications, performing routine procedures, and documenting in PointClickCare.

Qualifications

• Must have a valid LPN license.
• Experience in long-term care or skilled nursing is preferred.
• Ability to supervise nursing assistants.
• Willingness to work 12-hour shifts, with alternating holidays and weekends.

Responsibilities

• Monitor residents' condition and assess needs to provide quality care.
• Observe and interpret patients' symptoms and communicate findings to physicians.
• Perform routine procedures (blood pressure, injections) as ordered.
• Maintain charting in the PointClickCare system.
• Administer medications and treatments as ordered by physicians.
• Maintain hygiene and safety of residents and equipment.
• Provide emergency medical care when needed.
• Supervise nursing assistants to support care delivery.
• Foster a supportive environment for residents and their families.
• Attend monthly in-services to expand knowledge and skills.
• Work 12-hour shifts; schedule includes every other holiday and weekend.

The Avenue at North Ridgeville is a Post-Hospital Rehabilitation and Skilled Nursing Care Facility that offers a distinctive environment not commonly found in skilled nursing facilities. Our facility provides some of the area’s highest level of rehabilitation, nursing and medical support to individuals needing post-hospital care and services. The primary purpose of the LPN position is to provide direct nursing care to residents and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines and regulations.

Responsibilities

• Monitor patient’s condition and assess their needs to provide the best possible care and advice

• Observe and interpret patient’s symptoms and communicate them to physicians

• Perform routine procedures (blood pressure measurements, administering injections etc.)

• Perform skilled charting via the point click care charting system

• Adjust and administer patient’s medication and provide treatments according to physician’s orders

• Maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.)

• Provide instant medical care in emergencies

• Supervise nursing assistants

• Foster a supportive and compassionate environment to care for residents and their families

• Expand knowledge and capabilities by attending monthly in-services

• 12 hour shifts

• every other holiday

• every other weekend
