Job summary

The Avenue at North Ridgeville, a post-hospital rehabilitation and skilled nursing facility, is seeking a dedicated LPN to provide direct nursing care to residents and supervise nursing assistants. You will ensure care aligns with federal, state, and local standards while delivering compassionate support in a comfortable setting.

Typical duties include monitoring residents, administering medications, performing routine procedures, and documenting in PointClickCare.

Qualifications

Must have a valid LPN license.
Experience in long-term care or skilled nursing is preferred.
Ability to supervise nursing assistants.
Willingness to work 12-hour shifts, with alternating holidays and weekends.

Responsibilities

Monitor residents' condition and assess needs to provide quality care.
Observe and interpret patients' symptoms and communicate findings to physicians.
Perform routine procedures (blood pressure, injections) as ordered.
Maintain charting in the PointClickCare system.
Administer medications and treatments as ordered by physicians.
Maintain hygiene and safety of residents and equipment.
Provide emergency medical care when needed.
Supervise nursing assistants to support care delivery.
Foster a supportive environment for residents and their families.
Attend monthly in-services to expand knowledge and skills.
Work 12-hour shifts; schedule includes every other holiday and weekend.
