Your Role as a Licensed Practical Nurse (LPN)

As an LPN with Home Sweet Home, you will deliver hands-on skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN).

Primary Responsibilities

• Provide skilled, patient-centered nursing care based on individualized care plans


• Accurately document all visits, assessments, and treatments using electronic health records (EHR)


• Communicate effectively with patients, families, and the home health care team


• Ensure compliance with Arizona home health regulations and company policies


• Help patients meet their health goals through compassionate and professional nursing

Qualifications For Home Health LVN Jobs

• Current Arizona Licensed Practical Nurse (LPN) license in good standing.


• Graduate of an accredited vocational nursing program


• Minimum 1 year of nursing experience (home health experience preferred)


• Physical ability to assist and move patients as needed


• Strong communication and documentation skills


• Proficiency with electronic charting and care coordination software

Pay-Per-Visit : $45 USD - $55 USD
