Licensed Practical Nurse (LPN) – Home Care

Summary:

We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide high-quality nursing care to patients in their homes. The LPN will work under the supervision of a Registered Nurse (RN) or physician, following a care plan tailored to each patient's needs. This role involves clinical skills, communication, and the ability to work independently in a home environment.

Key Responsibilities:

* Provide direct nursing care to patients in their homes, including monitoring vital signs, administering medications, wound care, and other nursing interventions as prescribed.

* Observe, record, and report changes in the patient’s condition to the supervising RN or physician.

* Assist with personal care tasks such as bathing, dressing, and grooming as needed.

* Educate patients and families on disease processes, medications, and self-care techniques.

* Maintain accurate and timely documentation in accordance with agency policies and state regulations.

* Collaborate with healthcare team members to support patient care and ensure continuity.

* Follow infection control and safety protocols during patient visits.

* Support patients in maintaining their independence and quality of life at home.

Requirements:

* Current LPN license in \[State]

* CPR certification (BLS)

* One year of nursing experience preferred, home care experience a plus

* Strong clinical skills and judgment
