Job summary

Merakey seeks a Licensed Practical Nurse to join our Adult IDD team, splitting time between our Philadelphia facility and group homes in Philadelphia, Montgomery, and Bucks counties. You will travel to group homes to perform nursing assessments, monitor residents, and update care plans and MAR/Treatment logs.

The role involves providing clinical care, coordinating with healthcare professionals, and educating staff and families.

Qualifications

• Graduation from an accredited practical/vocational nursing program.
• Current license as an LPN in the Commonwealth of PA.
• Preferably, at least one year of experience as an LPN.
• Genuine interest in working with individuals with physical and developmental disabilities.
• Possession of a current and valid PA driver's license, along with reliable transportation.
• Physical capability to comfortably exert up to 50lbs occasionally, and/or up to 20lbs frequently, and/or up to 10lbs continuously.

Responsibilities

• Visit group homes, update MARs and Treatment Logs, conduct nursing assessments on residents to identify signs and symptoms of illness, and report findings to the nursing supervisor.
• Identify health issues and follow through to resolution, while documenting all actions.
• Review, transcribe, and administer physician's orders accurately.
• Collaborate with various healthcare professionals and specialists to coordinate care, while keeping the nursing supervisor informed.
• Provide health education to residents and their families.
• Conduct effective in-service training for staff to ensure compliance with protocols and procedures.
• Utilize the least restrictive approaches to behavior management and demonstrate knowledge of psychiatric emergencies.
• Respond effectively to emergency situations in a calm, constructive, and positive manner.

As a Licensed Practical Nurse (Group Home Visiting Nurse) at AVS Merakey, you'll provide direct nursing care to individuals with developmental and physical disabilities. Your responsibilities will include traveling to our group homes, clinical assessments, monitoring acuity, teaching staff on residents’ care plan and medical needs, and updating MAR and Treatment logs.

Earn:$32 / hour + Enhanced Pay Eligible and weekend / holiday shift differential where applicable.
