Job summary

Brooksby Village by Erickson Senior Living in Peabody, MA seeks an LPN/LVN to provide clinical care to residents in our neighborhood according to nursing standards and Erickson Senior Living guidelines. You will administer medications, update care plans, supervise care associates, document in the EMR, and collaborate with the RN and provider to support residents' physical, emotional, and spiritual well-being.

Minimum 1 year of experience preferred; active state license and CPR required.

Qualifications

• State-licensed LPN/LVN with active license.
• CPR certification required.
• Minimum 1 year experience in assisted living, home health, or long-term care preferred.
• Experience with electronic medical records preferred.
• Passion for senior care.

Responsibilities

• Develop relationships with residents and families while placing the resident first.
• Assess, observe, and document residents' clinical condition in the EMR; notify RN/provider as needed.
• Update or modify care plans as changes occur.
• Supervise and lead care associates.
• Administer medications timely, accurately, and per orders.
• Participate in meetings related to resident outcomes and care.
• Respond to residents, families, or responsible parties promptly and professionally.
• Participate in reviews of clinical indicators (falls, wounds, nutrition, antipsychotic use, etc.).
• Other duties as assigned by RN clinical leader.

Join our team as a Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN). The LPN/LVN is responsible for providing clinical care to residents in the neighborhood according to the nursing standards of practice, board of nursing, and Erickson Senior Living standards.

How you will make an impact

• Developing relationships with the residents and their families using a consistent caregiver approach supporting the resident's physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.
• Assessing, observing, and evaluating resident's clinical condition, document findings in the resident electronic medical record, and notify the provider and/or RN when necessary.
• Updating or modifying care plans as needed to reflect any changes to a resident's care and service needs.
• Supervising and leading care associates
• Administering medications timely, accurately, and according to orders.

• Responding to residents, family, or responsible parties in a timely and professional manner

What you will need

• Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
• Licensed by State as a Licensed Practical Nurse.
• CPR certification required.
• A passion to care for seniors and be a strong resident advocate.
• Experience with electronic medical records is preferred.
