Job summary

The Avenue at North Ridgeville is seeking compassionate Licensed Practical Nurses to provide direct resident care, monitor conditions, administer medications, and perform routine procedures in a post-hospital rehabilitation setting. You will chart in PointClickCare and supervise nursing assistants to support a high standard of care.

Qualifications

• Must have a valid LPN license.
• Experience in post-hospital rehabilitation and skilled nursing settings preferred.
• Clear communication and teamwork skills.

Responsibilities

• Monitor residents’ conditions and assess needs to provide quality care.
• Observe and report symptoms to physicians and document findings.
• Administer medications and perform routine procedures per orders.
• Chart via PointClickCare and ensure accurate medical records.
• Supervise nursing assistants and foster a compassionate environment.
• Maintain hygiene and safety in care areas and equipment.
• Provide immediate care in emergencies and participate in in-services.

Responsibilities

• Monitor patient’s condition and assess their needs to provide the best possible care and advice
• Observe and interpret patient’s symptoms and communicate them to physicians
• Perform routine procedures (blood pressure measurements, administering injections etc.)
• Perform skilled charting via the point click care charting system
• Adjust and administer patient’s medication and provide treatments according to physician’s orders
• Maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds etc.)
• Provide instant medical care in emergencies
• Supervise nursing assistants
• Foster a supportive and compassionate environment to care for residents and their families
• Expand knowledge and capabilities by attending monthly in-services
• 12 hour shifts
• every other holiday
• every other weekend

Qualifications

• Must have Valid LPN License
