What You Will Do
- Monitor and report patient progress and health changes to supervising RN through detailed observations, interviews, and psychosocial assessments within LPN scope of practice
- Deliver personalized nursing and care management, continuously evaluating and adjusting the care plan based on patient needs
- Coordinate care effectively by communicating within the interdisciplinary team and with external providers
- Practice in the community, traveling to patients' homes or care facilities, managing caseloads efficiently
- Provide continuity of care through education, supply coordination, and empathetic support
- Utilize technology, including tablets and EMRs, to maintain accurate and current records
- Empower patients and families through education and support, including end-of-life care discussions
Qualifications
Licenses and Certifications:
- Current NYS LPN license and registration required
- Valid driver’s license may be required based on operational needs
Education:
- High School Diploma or completion of LPN program required
Work Experience:
- Minimum one year of LPN experience preferred
- Bilingual skills may be required based on operational needs
- Home care/hospice experience preferred
- For CMHS: Experience in mental health and/or home care strongly preferred
- For Hospice: Role focused on specific service areas but cross-trained in all three hospice services
Pay Range per Visit: $50.68 - $54.07, if applying for the per diem per visit position.
Pay Range: USD $36.76 - $38.47 per hour
