Job summary

The McKendree Post Acute & Rehabilitation in Hermitage, TN is seeking dedicated LPNs to join our clinical team. This full-time role involves providing nursing care to residents, administering medications, and collaborating with RNs, CNAs, and other professionals to ensure high-quality care for seniors.

Qualifications

• Current LPN license in state of Tennessee or eligible to practice in Tennessee.
• CPR/BLS certification (or willing to obtain).
• Previous LTC or SNF experience preferred, but new grads welcome!

Responsibilities

• Provide nursing care to meet resident needs in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.
• Administer medications and treatments with precision and compassion.
• Collaborate with RNs, CNAs, and interdisciplinary teams.
• Support residents’ physical, emotional, and psychological well-being.
• Assist with resident comfort.
• Collect samples and monitor catheters. Provide wound care.
• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.
• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.
• Help coach staff as necessary.
