The role involves providing in-home nursing care, health assessments, and medication management for our clients. The LPN must meet background checks, licensing requirements, and provide necessary original documents such as CT Driver’s License/State ID and Social Security Card; fingerprints are required.

Qualifications

• Physical Forms (within past year)
• Must have an LPN certification and possess the original document of certification from a state approved training provider.
• Must provide original documents for CT Driver's License or CT State ID and original Social Security Card.
• Documentation of a physical and PPD taken within the past 12 months or be willing to obtain a physical and PPD test quickly.

Responsibilities

• Provide direct nursing services in the client’s home, including periodic health assessments, teaching and care coordination.
• Perform skilled intervention and medication management in home health settings.
• Deliver compassionate care to seniors and clients with disabilities, aligning with Utopia Home Care’s standards.
