Qualifications

• Must have an LPN certification from a state-approved provider.
• Must provide original CT Driver's License/State ID and Social Security Card.

Responsibilities

• Provide direct nursing services in the client's home, including health assessments and medication management.
• Coordinate care and perform skilled interventions as needed.
• Support patients and families with compassionate, patient-centered care.
