Job summary

Parkwood Health & Rehab in Louisville, KY is seeking compassionate LPNs to join our long-term care and skilled nursing team. Full-time and part-time roles available with shifts from 7am-3pm, 3pm-11pm, and 11pm-7am; a full-time day weekend option is also offered.

Competitive pay ($27-$31/hr) plus shift differentials and a generous sign-on bonus up to $10,000. We value dedicated, dependable LPNs who foster teamwork and resident dignity.

Qualifications

• Current LPN license in KY.
• CPR/BLS certification or willingness to obtain.
• LTC or SNF experience preferred; new grads welcome.
• Positive attitude, reliability, and strong clinical judgment.

Responsibilities

• Provide nursing care to meet resident needs per standards and orders.
• Administer medications and treatments with precision and compassion.
• Collaborate with RNs, CNAs, and interdisciplinary teams.
• Support residents’ physical, emotional, and psychological well-being.
• Assist with resident comfort.
• Collect samples, monitor catheters, and provide wound care.
• Maintain confidentiality and follow all policies and local requirements.
• Monitor vital signs and patient condition; coach staff as needed.

What You’ll Do:

• Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines
• Administer medications and treatments with precision and compassion
• Collaborate with RNs, CNAs, and interdisciplinary teams
• Support residents’ physical, emotional, and psychological well-being
• Assist with resident comfort.
• Collect samples and monitor catheters. Provide wound care.
• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.
• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.
• Help coach staff as necessary.

Requirements:

• Current LPN license in KY
• CPR/BLS certification (or willing to obtain)
• Previous LTC or SNF experience preferred, but new grads welcome!
