Job summary

Palms Home Care in Lakeland, FL is seeking a dedicated Home Health LPN to join our compassionate team. You will assess patient conditions, administer prescribed meds, dress wounds, and educate families on home health care procedures.

Coordinate with physicians and therapists to ensure quality care, recommend aids to improve daily living, and maintain meticulous charting and safety practices. A valid Florida LPN license in good standing is required, with a background screening per AHCA.

Qualifications
