Job summary

Palms Home Care is expanding and seeks a dedicated Home Health LPN to deliver patient-centered care in the home setting in Fort Myers, FL. You will chart observations, administer medications, and provide wound care while educating families on proper health practices.

The role requires a valid LPN license, at least 1 year in home health, and strong communication, organizational, and problem-solving skills. A flexible schedule and mileage reimbursement are available for full-time staff.

• Administer Physician-prescribed medication.
• Dress or redress wounds and monitor the healing progress.

• Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient's care plan.
