Job summary

Palms-Home-Care is seeking a dedicated Home Health LPN to support patients in their homes in Florida. You will chart observations, administer physician-prescribed medications, dress wounds, and educate families on proper home health care procedures.

The role emphasizes coordinating with physicians and therapists, recommending assistive devices, and delivering compassionate, patient-centered care with strong communication and organizational skills.

Qualifications

• Valid LPN license and in good standing.
• 1 year of Home Health experience required.
• Excellent knowledge of nursing care methods and procedures.
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to always follow them.
• A team player with excellent communication and interpersonal skills.
• Responsible and compassionate.
• Outstanding organizational and multi-tasking skills.
• Patient with excellent problem-solving skills.
