Responsibilities

The LPN in Home Health provides quality patient care and performs technical skilled care in the patient's home environment.

• Works within the scope of practice defined by state laws, following the plan of care, and coordinating with the healthcare team. May not alter the plan of care.
• Instructs patients' families in patient care and maintaining a healthy environment.
• Observes, records, and reports the patient's response to treatment and any changes to the supervising nurse or physician daily.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice
• Valid driver's license, vehicle insurance, and access to reliable transportation (vehicle or public transit)
• CPR certification required
