Job summary

Oak Park Place is seeking a Licensed Practical Nurse (LPN) to deliver thoughtful care based on each resident’s Plan of Care. You will monitor health changes, track progress, and communicate updates to the appropriate contacts.

The role emphasizes compassionate patient care, attention to detail, and collaboration with a caregiving team in a fast-paced environment. A valid LPN license is required to join our dedicated staff.

The Licensed Practical Nurse (LPN) is responsible for providing thoughtful care to residents and their families based on the personalized Plan of Care, monitoring and tracking changes in conditions and needs, and proactively updating the appropriate contacts on the plan of care, health changes, and concerns.
