Oak Park Place in Minnesota is seeking a Registered Nurse (RN) to provide thoughtful care to residents based on personalized Plan of Care. You will monitor changes in health status and communicate updates to the care team.

Registered Nurse (RN)

Responsibilities include providing thoughtful care to residents based on personalized Plan of Care, monitoring and tracking changes in the residents' conditions and needs, and proactively updating the appropriate contacts regarding the residents' plan of care, health changes, and concerns.

Requirements
