Oak Park Place is seeking a licensed practical nurse (LPN) to provide thoughtful, plan-driven care for residents and their families. You will monitor health changes, document updates, and communicate with the care team to ensure continuity of care in a supportive environment.

The role requires active LPN licensure, experience in nursing or related medical fields, and ability to thrive in a fast-paced setting.

The Licensed Practical Nurse (LPN) is responsible for providing thoughtful care to residents and their families based on the personalized Plan of Care, monitoring and tracking changes in conditions and needs, and proactively updating the appropriate contacts on the plan of care, health changes, and concerns.
