Job summary

Amedisys Home Health seeks a licensed practical nurse to join our team delivering in-home care across Jacksonville, FL. You will verify plans of care with RNs, coordinate with physicians, educate patients and caregivers, monitor vital signs, and perform medication reviews and select procedures in patients' homes.

We offer competitive base pay per visit ($34.87–$43.59 after orientation), comprehensive benefits, a 401(k) with company match, PTO, and family support programs.

Qualifications

• One year of clinical LPN experience required.
• Current CPR certification required.
• Current LPN license for the state(s) you are assigned to work.
• Valid driver's license and reliable transportation.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them if patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review all medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
