Job summary

Westlake Nursing and Rehabilitation Center is seeking compassionate, dependable LPNs to join our nursing team in Florida. You will provide high-quality care to seniors, administer medications, monitor residents, and support CNAs in a collaborative, person-centered environment.

Shifts include 7:00 AM–3:00 PM, 3:00 PM–11:00 PM, and 11:00 PM–7:00 AM; weekend/holiday availability may be required. CPR/BLS and an active FL LPN license are required.

Qualifications

• Current, active LPN license in Florida and valid CPR/BLS certification.
• Graduate of an accredited practical nursing program.
• Experience in long-term care or skilled nursing settings preferred.

Responsibilities

• Administer medications and therapies as prescribed.
• Monitor residents and report changes to RN/physician.
• Supervise and support CNAs in daily care.
• Maintain accurate documentation in EMR system.
• Assist with admissions, transfers, and discharges.
• Collaborate with physicians and interdisciplinary team members.
• Ensure CMS, state, OSHA, HIPAA, and facility policy compliance.
• Promote resident dignity, safety, and quality of life.
• Respond to resident needs and emergencies promptly.
• Participate in care planning meetings and quality initiatives.

Location: Westlake Nursing and Rehabilitation Center
Position: Licensed Practical Nurse (LPN)

Duties and Responsibilities

• Administer medications, treatments, and therapies as prescribed.
• Monitor and assess residents, reporting changes in condition to the RN and physician.
• Supervise and support Certified Nursing Assistants (CNAs).
• Maintain accurate and timely documentation in the electronic medical record (EMR).
• Assist with admissions, transfers, and discharges.
• Collaborate with physicians, therapists, social services, and other interdisciplinary team members.
• Ensure compliance with CMS, state, OSHA, HIPAA, and facility policies.
• Promote resident dignity, safety, and quality of life.
• Respond promptly to resident needs and emergency situations.
• Participate in care planning meetings, in-service education, and quality improvement initiatives.

Qualifications

• Current, active Licensed Practical Nurse (LPN) license in the State of Florida.
• Graduate of an accredited Practical Nursing program.
• Current CPR/BLS certification.
• Minimum of One-Year Long-term care or skilled nursing experience required.
• Strong communication, organizational, and critical thinking skills.
• Ability to work independently and as part of a multidisciplinary team.
• Experience with electronic medical records (EMR) is required.
