Job summary

Interim HealthCare Inc. is seeking a licensed practical nurse to join our home care and staffing team in Des Moines, IA. You will provide compassionate, patient-centered care, administer medications, monitor vitals, and document care plans in accordance with facility protocols.

Qualifications

• Active LPN license in the applicable state.
• Must be at least 18 years old.
• Previous LPN experience preferred.
• Maintain all required certifications and standards.

Responsibilities

• Assist patients with activities of daily living and provide compassionate, individualized care.
• Administer medications and treatments according to orders and protocols.
• Assist with ambulation, transfers, and range-of-motion exercises.
• Monitor and record vital signs including BP, pulse, temp, and respirations.
• Provide wound care and monitor healing.
• Care for PEG tubes and related feeding needs as applicable.
• Administer infusions and monitor responses.
• Conduct nursing assessments and adjust plans as needed.
• Complete accurate clinical documentation.
• Perform other nursing duties within the LPN scope.
• Assignment dependent: may act as charge nurse during scheduled shift.

Licensed Practical Nurse Job Responsibilities

• Assisting patients with activities of daily living (ADLs) and providing compassionate, individualized care
• Administering medications and treatments according to provider orders and facility protocols
• Assisting patients with ambulation, transfers, and range-of-motion exercises
• Monitoring and recording vital signs, including blood pressure, pulse, temperature, and respirations
• Providing wound care and monitoring wound healing
• Providing care for patients with PEG tubes and assisting with related feeding needs, as applicable
• Administering infusions and monitoring patients for responses or potential complications, as applicable
• Conducting nursing assessments and monitoring patients for changes in condition that may require adjustments to their plan of care, medications, or medical equipment
• Completing accurate and timely clinical documentation
• Performing other nursing duties as assigned within the LPN scope of practice
• Assignment dependent: Capacity to act as the charge nurse on the floor for the entirety of scheduled shift.

Qualifications

• Active Licensed Practical Nurse (LPN) license in South Dakota, or eligibility for licensure through reciprocity or multistate privileges, as applicable.
• Must be at least 18 years of age.
• Previous LPN experience highly preferred
• Must maintain all required certifications, qualifications, and professional standards
• Ability to provide compassionate, professional nursing care in a variety of healthcare settings
