Job summary

Interim HealthCare is seeking a Licensed Practical Nurse (LPN) to provide in-home and facility-based nursing support across our service areas. You will administer medications, monitor vitals, and assist with ADLs, transfers, and wound care under physician orders and facility protocols.

This PRN/Part Time/Full Time role offers flexible scheduling via our mobile app, on-call support, and benefits including health insurance and 401(k) eligibility.

Qualifications

• Active LPN license and eligibility for reciprocity
• Must be at least 18 years old
• Previous LPN experience preferred
• Maintain required certifications and professional standards

Responsibilities

• Assisting patients with activities of daily living (ADLs) and providing compassionate, individualized care
• Administering medications and treatments according to orders and facility protocols
• Assisting patients with ambulation, transfers, and ROM exercises
• Monitoring and recording vital signs (BP, pulse, temperature, respirations)
• Providing wound care and monitoring healing
• Care for patients with PEG tubes and related feeding needs
• Administering infusions and monitoring for responses or complications
• Conducting nursing assessments and adjusting care plans as needed
• Completing accurate clinical documentation
• Performing other nursing duties within LPN scope of practice
• Capacity to act as charge nurse during shifts

Licensed Practical Nurse Job Responsibilities

• Assisting patients with activities of daily living (ADLs) and providing compassionate, individualized care
• Administering medications and treatments according to provider orders and facility protocols
• Assisting patients with ambulation, transfers, and range-of-motion exercises
• Monitoring and recording vital signs, including blood pressure, pulse, temperature, and respirations
• Providing wound care and monitoring wound healing
• Providing care for patients with PEG tubes and assisting with related feeding needs, as applicable
• Administering infusions and monitoring patients for responses or potential complications, as applicable
• Conducting nursing assessments and monitoring patients for changes in condition that may require adjustments to their plan of care, medications, or medical equipment
• Completing accurate and timely clinical documentation
• Performing other nursing duties as assigned within the LPN scope of practice
• Assignment dependent: Capacity to act as the charge nurse on the floor for the entirety of scheduled shift.

Qualifications

• Active Licensed Practical Nurse (LPN) license in South Dakota, or eligibility for licensure through reciprocity or multistate privileges, as applicable.
• Must be at least 18 years of age.
• Previous LPN experience highly preferred
• Must maintain all required certifications, qualifications, and professional standards
• Ability to provide compassionate, professional nursing care in a variety of healthcare settings
