Job summary

Alternate Solutions Home Health – Columbus is seeking an LPN I to provide comprehensive skilled care in patients' homes, following physician orders and RNCM guidelines. Responsibilities include patient education, accurate documentation, and ensuring safety within scope of practice.

The position is a Monday–Friday schedule (8:30 AM – 5:00 PM) with requirement of active LPN license in Ohio and valid driving access.

Qualifications

• Minimum of one year of experience as an LPN in an acute care setting is required; two years is preferred.
• Home care experience is desired.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the physician.
• Observes and monitors patient conditions and notifies RNCM of changes.
• Administers medication as prescribed by the physician.
• Counsels patient and family on disease/injury processes and plan of care.

Summary

The Licensed Practical Nurse (LPN) I provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM. The LPN I educates patients and their family members and ensures the safety of the patient. The LPN I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN I is responsible for all practices and duties within the scope of practice as outlined by the state.

Key Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the attending physician
• Performs care by following order sets and goals made by RN
• Observes and monitors patient conditions
• Notifies RNCM of any changes in patient conditionAdministers medication as prescribed by the physician
• Counsels patient and family on the disease/injury processes and how to manage
• Incorporates patient and family in development of plan of care
• Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions
• Listens to patient and family members
• Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
• Documents all aspects of treatment, assessments, and patient education
• Maintains active LPN license in state of employment

Qualifications

• Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred. Home care experience is desired.
