Job summary

Alternate Solutions Health Network is seeking a Licensed Practical Nurse (LPN) I to provide skilled nursing care in patients' homes in the Columbus area. You will follow physician orders, use RN-created order sets, and educate patients and families on care plans.

The role emphasizes documentation accuracy, adherence to Medicare/Federal guidelines, and collaboration with physicians and co-workers. Schedule is Monday through Friday, 8:30 AM to 5:00 PM.

Qualifications

• Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and co-workers.
• Detail-oriented and observant with a disciplined work ethic.

Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Performs care by following order sets and goals made by RN.
• Observes and monitors patient conditions.
• Notifies RNCM of any changes in patient condition.
• Administers medication as prescribed by the physician.
• Counsels patient and family on disease/injury processes and management.
• Develops and documents plan of care with patient/family involvement.
• Documents all clinical treatment, assessments and education in accordance with agency protocol and Medicare/Federal guidelines.
