Compassus is seeking a Home Health Licensed Practical Nurse (LPN) to provide compassionate in-home care, empowering patients to live safely at home and participate in activities they value with their families. The role emphasizes education, assessment, and skilled nursing interventions delivered in patients' homes.

The LPN will support care plans, monitor health status, administer medications as allowed, and collaborate with a multidisciplinary team to optimize outcomes.

Qualifications

• Graduate of an accredited school of Practical Nursing.
• Current state LPN license.
• Two years of LPN experience; home health preferred.

Responsibilities

• Support patient recovery and enable at-home care.
• Educate patients and families on disease management and self-care.
• Assist with home visits, documentation, and care coordination.
