Job summary

Ascension at Home together with Compassus is seeking a Home Health Licensed Practical Nurse (LPN) to support patient recovery at home and empower families in disease management.

The role requires an LPN license, current CPR, a valid driver’s license, and at least two years of clinical care experience, with home health experience preferred.

Qualifications

• Graduate of an accredited school of Practical Nursing.
• Current state LPN license.
• Current driver’s license and CPR certification.
• TB screen and hepatitis consent/declination completed.
• Two years of clinical care experience, home health preferred.

Responsibilities

• Support patient recovery and enable at-home care, helping patients spend time with loved ones.
• Educate patients and families to achieve healthcare goals and disease management.
