Job summary

Bradford Health Services, The Willows (Fletcher, NC), seeks a Licensed Practical Nurse to provide medical case management for clients with substance use and co-occurring disorders. The role includes medication management, documentation, and on-site medical support in a trauma-informed program.

Minimum qualifications include an Associate degree in nursing and NC LPN licensure; experience in substance abuse/mental health is preferred. The position is full-time and onsite at our Fletcher campus.

Qualifications

• Current North Carolina state licensure as an LPN
• Onsite full-time availability

Responsibilities

• Conduct new client intakes
• Manage all client medications accurately
• Administer medications as needed
• Document all medication administration promptly
• Notify and update family on medical concerns
• Support direct care staff with med administration and documentation
• Provide routine medical check-ins and serve as primary medical provider on base
• Participate in rotating on-call schedule
• Offer medical consultation for base/field scenarios when on call
• Assist campus RN and keep client charts up to date

Education

Associate degree in nursing

NC LPN license

Bachelor’s degree in nursing (preferred)

Responsibilities

• Conduct new client intakes
• Participate in accurate and detailed management of all client medications
• Participate in medication administration as needed
• Ensure effective documentation of all medication administration in assigned timeframe
• Communicate with and update client’s parent/ family, if applicable, on any medical concerns
• Provide medication administration support to direct care staff and ensure that proper medical documentation and quality control occurs
• Provide medical check-ins with clients and be the primary medical care provider while on base
• Participate in a rotating on call schedule
• Be available for medical consultation/expertise for all base/field scenarios when on call
• Support campus RN as required
• Collaborate with campus RN to keep client charts up to date

Minimum Qualifications

• Associate degree in nursing
