Job summary

Elite Home Health, part of the LHC Group and a member of the Optum family, is hiring a Physical Therapist Assistant to deliver therapy in patient homes under the supervision of a PT. You will implement the plan of care and guide patients through therapeutic activities.

You will educate patients and families, assist with exercises, monitor progress, and collaborate with the health care team to ensure compassionate, high-quality care across diverse home settings.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
• Available to work a weekend rotation
• CPR certification or ability to complete within 90 days of hire

Responsibilities

• Follow state-specific laws governing home care PT; perform only those procedures authorized by the supervising PT
• Instruct and aid patients in active and passive exercises, gait, ADL, and transfer training
• Observe, record, and report patient responses to treatment to the supervising PT and care team
• Educate patients, families and care team on therapy within the PT assistant scope
• Function in diverse home environments with respect to all clients' backgrounds
