Job summary

Hartford HealthCare at Home is seeking a qualified Physical Therapist Assistant to support the clinical team under a Physical Therapist's supervision. You will administer treatments, assist in assessments, and help implement care plans in patients' homes across Connecticut.

Responsibilities include performing delegated tests, delivering interventions, documenting progress, and educating patients and caregivers.

Qualifications

• Knowledge of principles and practices of Physical Therapy.
• Excellent communication and organizational skills.
• Excellent interpersonal skills; ability to interact with patients, families and co-workers.
• Ability to deal effectively with stress and ability to exercise independent judgment.

Responsibilities

• Assist the Physical Therapist with patient assessment by performing delegated duties such as testing for joint range of motion, muscle strength, and mobility status, identifying rehab precautions.
• Recognize physiological and age-related changes in the patient; consult with the Physical Therapist and other Health Care Team members.
• Perform therapeutic techniques as delegated by the Physical Therapist and deliver patient care using interventions such as heat, cold, electrical stimulation, and therapeutic exercise.
• Communicate patient progress to the Physical Therapist.
• Provide patient and caregiver education regarding treatment, precautions, and equipment use as delegated by the Physical Therapist.
• Complete all documentation including evaluation, care plan and goals, patient status and progress, visits, discharge summary, and care transition plan.
• Assist with patient transition planning and discharge planning including recommendations to community resources.
