Job summary

Hartford HealthCare at Home is seeking an experienced Physical Therapist to provide home-based therapy to patients in Connecticut. You will evaluate patients, develop treatment plans, and implement care to maximize independence and function in the home setting.

Hours are flexible with full-time, part-time, or per diem options; benefit-eligible at 24+ hours/week. The role emphasizes high-quality, patient-centered outcomes and collaboration with a multidisciplinary team.

Qualifications

• PT experience is required.
• Graduate from an accredited Physical Therapy program (BS or entry-level master’s).
• Active CT PT license and CPR certification required at start.

Responsibilities

• Evaluate, plan, and implement physical therapy for homebound patients.
• Evaluate and determine treatment strategies, deliver and/or delegate services.
• Enhance patient function through exercise, manual therapy, and therapeutic modalities.
• Assess strength, mobility, pain, gait, balance, and activities of daily living.
