Job summary

Responsibilities

• Manage a patient caseload on a daily basis.
• Under supervision of a Physical Therapist, perform treatment modalities per plan of care.
• Implement care plans to provide safe, efficient quality home care.
• Coordinate patient care with Agency personnel and community resources.
• Consult with PT regarding outcomes of home visits and document findings.
