• Develop individualized care plans based on findings and physician orders.
• Provide skilled interventions including exercise, gait training, and balance activities.
• Use standardized assessments to measure progress and outcomes.
• Collaborate with OT, SLP, nursing, physicians, and the rehab team.
• Document patient progress accurately and timely.
• Participate in interdisciplinary meetings and quality improvement initiatives.
• Educate residents and families on safety and home programs.
