Physical Therapist – Skilled Nursing

Join a great rehab team at Valley Village Care Center!

We are looking for a licensed Physical Therapist to join our established rehabilitation team at Valley Village Care Center in North Hollywood. This is a great opportunity for a PT who enjoys working collaboratively, values quality patient care, and wants to be part of a supportive and positive learning environment.

Our rehab department has a strong team in place, making this an excellent setting for an experienced therapist or someone looking to continue developing their clinical skills with the support of knowledgeable rehabilitation professionals.

Responsibilities:

• Evaluate residents' mobility, strength, balance, functional limitations, and rehabilitation needs

• Develop individualized plans of care based on clinical findings, resident goals, and physician orders

• Provide skilled interventions including therapeutic exercise, functional mobility, transfer and gait training, balance activities, and other appropriate treatments

• Utilize standardized assessments to measure progress and demonstrate functional outcomes

• Monitor resident progress and modify treatment plans based on clinical response and changing needs

• Collaborate closely with OT, SLP, nursing, physicians, and the interdisciplinary team to provide coordinated care

• Educate residents, families, and caregivers regarding safety, mobility, therapeutic interventions, and home programs

• Complete accurate and timely clinical documentation

• Participate in interdisciplinary meetings, staff education, and quality improvement initiatives

• Contribute to a collaborative rehab department focused on excellent resident outcomes

Qualifications:

• Current California Physical Therapist license

• Skilled nursing or post-acute experience preferred, but we are open to clinicians who are eager to learn and grow

• Strong clinical judgment, communication, and documentation skills

• Ability to work collaboratively as part of an interdisciplinary team

• Commitment to providing compassionate, resident-centered care
