Valley Village Care Center LLC in North Hollywood, California seeks a licensed Occupational Therapist to join our skilled nursing team on a per diem basis. You will evaluate and treat residents with physical, cognitive, and psychosocial impairments to help them achieve independence and quality of life.

• Current license to practice as an Occupational Therapist in the state of California.

Mansioni

• Evaluate residents' physical, cognitive, and psychosocial abilities and functional limitations using standardized assessments
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
• Provide skilled occupational therapy interventions, including therapeutic exercise, activities of daily living training, cognitive retraining, and environmental modifications
• Monitor and document resident progress and adjust treatment plans as necessary
• Communicate with interdisciplinary team members, including physicians, nurses, physical therapists, and speech therapists, to coordinate care and ensure optimal outcomes for residents
• Educate residents and family members on occupational therapy goals, treatment plans, and home exercise programs
• Maintain accurate and timely documentation of occupational therapy services provided
• Participate in team meetings, in-service education, and quality improvement initiatives
• Maintain current knowledge of occupational therapy practice and relevant regulations
