Job summary

TEEMA Solutions Group is seeking a compassionate and licensed Physical Therapist (PT) in California to evaluate, develop, and implement individualized treatment plans that restore and improve patients' physical function and mobility. Qualifications

• DPT or foreign equivalent required.
• Active PT license in state of practice.
• Current BLS/CPR certification.
• Two years of PT experience preferred.
• Strong clinical assessment, communication, and documentation skills.
• Ability to collaborate within an interdisciplinary healthcare team.

Responsibilities

• Evaluate patients and perform comprehensive physical therapy assessments.
• Develop individualized treatment plans based on patient needs and clinical findings.
• Provide therapeutic interventions using evidence-based treatment techniques and protocols.
• Monitor patient progress and modify treatment plans as appropriate.
• Establish individualized care plans and discharge plans while communicating recommendations to patients and the healthcare team.
• Maintain accurate, timely, and complete clinical documentation.
• Collaborate with physicians and interdisciplinary team members to optimize patient outcomes.
• Supervise Physical Therapist Assistants and support staff as assigned.
• Ensure compliance with departmental policies, safety standards, and regulatory requirements.
• Perform other job-related duties as assigned.
