Job summary

vTech Solution is seeking a licensed Physical Therapist to provide skilled therapy services, perform comprehensive assessments, develop plans of care, and coordinate with interdisciplinary teams to promote rehabilitation.

You will supervise PTAs and home health aides, ensure regulatory compliance, document accurately, manage patient schedules, and engage in ongoing professional development and community outreach.

Qualifications

• Licensed Physical Therapist in the state of practice.
• Knowledge of federal and state regulations, organizational policies, and APTA guidelines.
• Strong clinical assessment and treatment planning skills.
• Experience with patient care coordination and interdisciplinary collaboration.
• Proficiency in documentation, billing, and use of electronic medical records.
• Excellent communication and interpersonal skills.
• Ability to manage patient schedules and adjust to changing demands.

Responsibilities

• Monitor and amend patient visit schedules to maximize productivity.
• Perform comprehensive physical therapy assessments including psychosocial, educational, and safety parameters.
• Develop and document individualized plans of care with patient-specific goals.
• Provide skilled physical therapy treatments and patient/caregiver education.
• Coordinate patient care with physicians, nurses, caregivers, rehab services, and students.
• Supervise and direct clinical activities of PTAs, PT students, and home care aides.
• Complete and coordinate documentation and paperwork to ensure compliance with regulations and support billing processes.
• Participate in in-service training and promote rehabilitation services to customers and the community.
• Conduct patient-related health assessments and procedures such as staple/suture removal and wound care.
• Communicate changes in patient care schedules promptly to relevant staff.
• Complete OASIS time points and ensure accuracy of clinical notes and documents.
• Follow agency protocols for visit documentation and billing processes.
• Assess patient needs for home care aide services and coordinate plans of care as necessary.
• Coordinate safe patient discharges and transfers with appropriate documentation.
• Report patient incidents to quality improvement specialists in a timely manner.
• Participate in peer review audits and provide/receive professional feedback.
