Musculoskeletal Care Rehab Clinic in Fresno, CA is seeking a full-time Licensed Physical Therapist to deliver high-quality care to a diverse patient population. You will conduct evaluations, create tailored treatment plans, and guide patients through rehabilitation to improve mobility and quality of life.

The role requires a CA PT license, a willingness to perform in-person and telehealth evaluations, and a collaborative, patient-centered approach within a multidisciplinary team.

Formación

• Master's degree in Physical Therapy or Doctor of Physical Therapy (DPT) from an accredited program.
• Active and unrestricted California PT license.
• Willingness to provide both in-person and telehealth evaluations and services.
• Commitment to working with a diverse patient base in a fast-paced environment.
• Excellent communication, teamwork, and customer-service mindset.

Responsabilidades

• Conduct initial evaluations and assess patients' physical abilities.
• Design and implement individualized treatment plans.
• Monitor progress and adjust plans to optimize recovery.
• Document care in EHR with SOAP notes and care plans on time.
• Educate patients and families on therapy techniques and timelines.
• Collaborate with interdisciplinary teams for holistic care.
• Meet KPIs and contribute to a positive clinic culture.
• Model patient-centered care and teamwork.
