Qualifications

• Licensed in California to practice physical therapy.
• Ability to conduct in-person and telehealth evaluations and treatments.
• Demonstrated teamwork and strong patient service orientation.

Responsibilities

• Perform comprehensive PT evaluations for all ages and conditions.
• Create, implement, and adjust individualized treatment plans.
• Reassess progress with standardized measures and document in the EHR.
• Educate patients and families on recovery strategies and self-management.
• Collaborate with multidisciplinary team to coordinate care.
• Maintain productivity targets while upholding quality and safety.
