Concept Rehab, Inc. is seeking a licensed Physical Therapist Assistant (PTA) to join our Skilled Nursing Facility (SNF) team in Avon, OH. This full-time/PRN role focuses on improving mobility, safety, and quality of life for older adults in a post-acute setting.

You will work with a therapist-led team in a supportive environment that values clinical excellence, ongoing education, and leadership development opportunities.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Ohio PTA license.
• Strong communication and interpersonal skills.
• Ability to work collaboratively within an interdisciplinary healthcare team.
• Commitment to patient-centered care.

Responsibilities

• Provide physical therapy treatments in accordance with individualized care plans.
• Help patients improve mobility, strength, balance, endurance, and functional independence.
• Communicate patient progress and recommendations to the supervising Physical Therapist and interdisciplinary care team.
• Collaborate with nursing staff, physicians, families, and caregivers.
• Participate in care planning and discharge planning activities.
• Educate patients, families, and facility staff on therapy programs and safety strategies.
• Maintain timely and accurate electronic documentation and billing records.
• Assist with restorative and functional maintenance programs.
• Recommend equipment and treatment interventions that support successful patient outcomes.
• Support a positive rehabilitation environment focused on quality care and service excellence.

Requirements
