EmpowerMe Wellness is seeking licensed therapists to join the EmpowerMe 360 program, delivering therapy, wellness, and medical care to seniors in partnered living communities. You will assess residents, design personalized plans, and collaborate with a multidisciplinary team to enhance independence and safety.

Responsibilities

• Provide therapy and wellness services for older adults as part of EmpowerMe 360
• Conduct assessments, create personalized care plans, and deliver evidence-based interventions with a team
• Support care coordination with wellness check-ins, caregiver communication, and telehealth support
• Maintain professional and ethical standards aligned with company initiatives
• Contribute to program development and continuous quality improvement

• Conduct assessments, create personalized care plans, and deliver evidence-based interventions as part of a collaborative team
• Support care coordination with wellness check-ins, caregiver communication, and telehealth support
• Maintain professional and ethical standards in line with company initiatives
• Contribute to program development and continuous quality improvement
• Help residents maximize independence, confidence, and satisfaction
