Job summary

EmpowerMe Wellness is seeking qualified therapists to join our EmpowerMe 360 program, focused on reconnecting therapy, wellness, and medical care for seniors in community settings.

You will deliver therapy services, participate in care planning, and collaborate with a multidisciplinary team to enhance residents’ independence and safety while building strong community relationships.

Qualifications

• Current professional license in your therapy discipline (PT, PTA, OT, COTA, or SLP).
• Passion for empowering seniors to live their best lives.
• Strong clinical skills with a collaborative spirit.

Responsibilities

• Contribute to program development and continuous quality improvement.
