Job summary

Breezy Hills Rehabilitation and Care Center in Lakeland, Florida, is seeking a Licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. You will work under the direction of a licensed Physical Therapist to provide therapy services that help residents improve mobility, restore function, and achieve independence.

Responsibilities include implementing treatment plans, delivering exercises and gait training, documenting progress, and communicating with the care team.

Qualifications

  • Current, unrestricted Florida Physical Therapist Assistant (PTA) license.
  • Graduate of an accredited Physical Therapist Assistant program.
  • Minimum one year of experience in a skilled nursing, rehabilitation, or long-term care setting preferred.
  • Strong communication, interpersonal, and documentation skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Passion for providing exceptional resident-centered care.

  • Assist the Physical Therapist in implementing individualized treatment plans designed to improve residents' mobility, strength, balance, and functional independence.
  • Deliver therapeutic exercises, gait training, transfer training, and other treatment interventions as directed by the supervising Physical Therapist.
  • Monitor resident progress and accurately document treatments, responses, and outcomes in accordance with facility policies and regulatory guidelines.
  • Communicate resident progress, concerns, and changes in condition to the supervising Physical Therapist and interdisciplinary care team.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Ensure resident safety by following established infection prevention and safety protocols.
  • Maintain a clean, organized, and well-stocked therapy department.
  • Inspect therapy equipment and promptly report malfunctioning or unsafe equipment.
  • Utilize appropriate personal protective equipment (PPE) and adhere to OSHA and facility safety standards.
  • Demonstrate professionalism, compassion, and excellent customer service when interacting with residents, families, and team members.
  • Participate in interdisciplinary meetings and contribute to resident-centered care planning.
