Job summary

Breezy Hills Rehabilitation and Care Center in Lakeland, FL is seeking a Licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. You will work under the direction of a licensed Physical Therapist to help residents improve mobility and independence.

The position offers a full-time schedule with pay ranging from $28–$38 per hour based on experience, along with benefits and a supportive, team-oriented work environment.

Qualifications

  • Current, unrestricted Florida PTA license.
  • Graduate of an accredited Physical Therapist Assistant program.
  • Minimum one year of experience in a skilled nursing, rehabilitation, or long-term care setting preferred.

Responsibilities

  • Assist the Physical Therapist in implementing individualized treatment plans designed to improve residents' mobility, strength, balance, and functional independence.
  • Deliver therapeutic exercises, gait training, transfer training, and other treatment interventions as directed by the supervising Physical Therapist.
  • Monitor resident progress and accurately document treatments, responses, and outcomes in accordance with facility policies and regulatory guidelines.
  • Communicate resident progress, concerns, and changes in condition to the supervising Physical Therapist and interdisciplinary care team.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Ensure resident safety by following established infection prevention and safety protocols.
  • Maintain a clean, organized, and well-stocked therapy department.
  • Inspect therapy equipment and promptly report malfunctioning or unsafe equipment.
  • Utilize appropriate PPE and adhere to OSHA and facility safety standards.
  • Demonstrate professionalism, compassion, and excellent customer service when interacting with residents, families, and team members.
  • Participate in interdisciplinary meetings and contribute to resident-centered care planning.

Breezy Hills Rehabilitation and Care Center is seeking a compassionate and motivated Licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. In this role, you'll work under the direction of a licensed Physical Therapist to provide exceptional therapy services that help residents improve mobility, restore function, and achieve their highest level of independence.


Key Responsibilities
