Responsabilidades

• Evaluate medication orders for drug appropriateness, dosage, interactions and administration.
• Ensure dispensed medications are correctly supplied and labeled.
• Recommend therapy changes to physicians as needed.
• Maintain patient and facility databases including medical history and lab data.
• Investigate and resolve medication-related queries.
• Communicate with nursing staff, physicians and health team members about medications.
