Job summary

Pathways to Life, Inc. in Nags Head, NC seeks a Licensed Outpatient Therapist to provide individual, family, and group therapy across age groups.

You will manage outpatient caseloads, complete diagnostic assessments, and collaborate with families, schools, and medical providers to deliver person-centered care. The ideal candidate will hold an active NC license, have strong documentation skills, and be committed to HIPAA-compliant, ethical practice in a multidisciplinary team environment.

Qualifications

• Master’s degree in a behavioral health field.
• Active North Carolina clinical license (LCSW, LCSWA, LCMHC, LCMHCA, or equivalent).
• Experience with outpatient therapy, assessments, and treatment planning.
• Ability to maintain HIPAA-compliant records.

Responsibilities

• Provide individual, family, and group therapy based on each member’s clinical needs and treatment goals.
• Maintain an assigned outpatient therapy caseload.
• Complete Diagnostic and Comprehensive Clinical Assessments and reassessments.
• Develop and update Person-Centered Plans and treatment goals with members and families.
• Submit assessments for review within 48 hours and make corrections within 24 hours.
• Produce accurate clinical notes within agency timelines.
• Assist with authorizations and ensure required information is complete.
• Maintain tracking information on assessments, plans, and services.
• Coordinate care with families, schools, medical providers, and community supports.
• Support clinical teams in goals and coordinated interventions.
• Provide crisis assessments and intervention when needed.
• Maintain complete, confidential clinical records.
• Follow HIPAA, ethics, and state/payer requirements.
• Maintain licensure and complete continuing education.
• Participate in supervision, meetings, training, and quality-improvement activities.
• Remain flexible with scheduling.
